Each day your sales and BDC team get an email outlining actions they need to perform as part of their daily plan.
If you are seeing leads that the work was already done on but is still showing up on this list, the reason is probably because the action was not marked completed in the CRM.
To do so, in the lead, when you perform the action, make sure you check the "Action" box
Not it will load a list of all open actions for that lead.
Simply pick the action you are doing before you add your note.
This will mark the action as complete and make sure it does not show up in the report.
